Choose the plan that fits your healthcare needs and budget.&nbsp;• &nbsp; Samuel’s medical benefits are designed to help you and your family stay healthy and easily access proper care.&nbsp;&nbsp;How It Works• &nbsp; Most team members can choose from three medical plan options: Medical plan 1: HDHP, Medical Plan 2: HD PPO, and Medical Plan 3: Premium PPO.&nbsp;&nbsp;• &nbsp; All three offer in- and out-of-network benefits and are PPO plans, which means you do not have to go through a primary care physician to get medical services.&nbsp;&nbsp;&nbsp;For more details, see the&nbsp;Medical Plan Comparison Chart&nbsp;below.

You can enroll or change your medical plan election as a new hire, at annual enrollment or with a qualified life event.&nbsp;&nbsp;You have a choice of three coverage tiers:• &nbsp; Employee only, Employee + 1 dependent, and Family&nbsp;• &nbsp; All plans are administered by UnitedHealthCare (UHC).&nbsp;• &nbsp; All plans cover in-network preventive care at no cost to you and cover the same medical and behavioral health services and prescription drugs. Where the plans differ, is in your cost for coverage (the premium), and how much you pay if and when you receive other care for your co-pays, deductibles and out-of-pocket costs.&nbsp;&nbsp;• &nbsp; Another key difference: only Medical Plan 1: HDHP includes a Health Savings Account (HSA), and it includes HSA contributions from Samuel. Overview

Who is eligible?U.S. Employees, scheduled to work 30 or more hours per week (excludes temporary employees).&nbsp;Your costsSamuel pays the larger share of your overall health care costs. You pay your share in different ways:<ul><li dir="ltr">Premium contributions: you pay a portion of the medical insurance premium through pretax deductions from your pay. The amount depends on the plan you choose and your coverage tier.</li><li dir="ltr">&nbsp;</li><li dir="ltr">Copay: the set amount you pay for a covered service; for example, $15 for a doctor's visit or $6 for a prescription.</li><li dir="ltr">&nbsp;</li><li dir="ltr">Deductible costs: The set amount you pay for a health service before your plan starts paying.</li><li dir="ltr">&nbsp;</li><li dir="ltr">Out-of-pocket costs: the cost you will pay for covered care. If you reach this amount, your plan pays 100% after that.&nbsp;</li></ul>
