Because you care about those who depend on you financially, Samuel Offers Life and AD&amp;D insurance benefits – and peace of mind.&nbsp;&nbsp;How it Works• &nbsp;Life insurance pays a cash benefit to your beneficiary(ies) if you die.&nbsp;• &nbsp;Accidental Death and Dismemberment (AD&amp;D) insurance pays additional benefits if death is caused by an accident, or if you have certain serious injuries from an accident.&nbsp;&nbsp;• &nbsp;Samuel Provides Basic Life and AD&amp;D coverage at no cost to you. You have the option to supplement the basic coverage with additional coverage at your own cost.&nbsp;

Who is eligible?

• Name your beneficiary(ies) for each plan at connect.samuel.com. You are automatically the beneficiary for spouse and child life insurance.&nbsp;• If evidence of insurability is required, you will need to complete it by logging into&nbsp;<a href="https://www.lincolnfinancial.com/public/individuals">Lincolnfinancial.com</a>. If you are a first-time user, you will need the company code:&nbsp;LF1500SAM.• In the event you need to file a life or accident claim, please reach out to your HR representative to initiate this process or reach out to your benefits team at benefits.usa@samuel.com.&nbsp;&nbsp;&nbsp;

What to do

Basic Life and AD&D

For you• Up to 5 times your annual earnings. • $500,000 maximum. • Evidence of insurability may be required. • The employee guaranteed issue amount is $300,000. This is the amount a member may elect up to during the first 31 days of employment, without any medical questions.&nbsp; • Any amount above the guaranteed issue will require medical evidence of insurability. Any amount elected after the first 31 days of employment will require medical evidence of insurability.&nbsp;&nbsp;&nbsp;For your Spouse/Domestic Partner• Only available if employee life is elected.• Up to 50% of employee amount ($250,000 maximum).• Evidence of insurability may be required.• The Spouse guaranteed issue amount is 50% of employee's election, up to $50,000.• Any amount above the guaranteed issue will require medical evidence of insurability.
