Your Samuel medical plan provides a network of professionals who can help with anxiety, depression, family conflicts, substance abuse and other challenges.
How It Works
If you are enrolled in a Samuel medical plan, you can access a range of behavioral health services – both inpatient and outpatient – through your medical plan carrier, UnitedHealthCare (UHC).
Whether or not you have Samuel medical coverage, our EAP offers counseling and other behavioral health services, at no cost, to all team members and their families.
Who is Eligible?
Team members and dependents enrolled in a Samuel medical plan have coverage for behavioral services through UnitedHealthCare. For those not enrolled in Samuel’s medical plan, team members and dependents can utilize EAP.
Your Costs
After meeting your annual medical plan deductible, you will pay your portion of your coinsurance for in-network behavioral health services.
What to Do
Be sure to present your medical plan ID card when using any behavioral health service.
If you are not enrolled in Samuel’s medical plan, visit guidanceresource.com to review counseling and other behavioral health services.
